Let us deliver your MONTHLY TICKET to your home. Just complete our "Ticket-By-Mail" application and we'll take care of the rest. The ticket will then be automatic each month and hassle-free.
All tickets will be mailed out eight working days before the end of each month. An invoice and a return envelope will be enclosed with your ticket. Your payment should be received by the fifth working day of the following month.
When going on vacation or if you just wish to temporarily stop service, just let us know. Ask any Ticket Agent for a Cancellation Request. The monthly ticket can be stopped for one month or as long as needed. The invoice will also have a section to change the service.
Print and complete the application below and return it one month before you want your first ticket delivered. Don't waste any more of your valuable time; let the mailman deliver your ticket.
Complete the application by filling it out online by clicking the fields and printing it or just printing it and manually filling it out. Then mail it to us at 503 N Carroll Ave., Michigan City, IN 46360-5026 or leave your application with a ticket agent.
